Care Manager
Position Purpose: Support members through comprehensive clinical assessments, personalized care planning, and continuous engagement to improve health outcomes.
Hybrid Role: Connect with members through phone calls, virtual visits, or in-person check-ins to monitor progress, provide education, and adjust care plans as needed.
Education: Master's in Behavioral Health or Social Work, or a Nursing Degree and 2-4 years of related experience
License/Certification: Licensed Master’s-level BH professional (e.g., LCSW, LMSW, LMFT) or RN with BH experience, per Florida requirements.

Care Manager, Behavioral Health
Position Purpose: Manages complex care for members with mental health and substance use needs, delivering personalized plans and education to support quality, cost-effective outcomes.
Hybrid Role: Connect with members through phone calls, virtual visits, or in-person check-ins to monitor progress, provide education, and adjust care plans as needed.
Education: Master's in Behavioral Health or Social Work, or a Nursing Degree and 2-4 years of related experience
License/Certification: Licensed Master’s-level BH professional (e.g., LCSW, LMSW, LMFT) or RN with BH experience, per state requirements.

Care Manager, Obstetrics (OB/GYN)
Position Purpose: Supports pregnant members through comprehensive assessments, personalized care planning, and ongoing engagement to promote healthy pregnancies and postpartum outcomes.
Hybrid Role: Connect with members through phone calls, virtual visits, or in-person check-ins to monitor progress, provide education, and adjust care plans as needed.
Education: Master's in Behavioral Health or Social Work, or a Nursing Degree and 2-4 years of related experience
License/Certification: Licensed Master’s-level BH professional (e.g., LCSW, LMSW, LMFT) or RN with BH experience, per state requirements.
