Community Resource Coordinator (Level II)
Position Purpose: Connects members to community resources, supports care teams, and provides health education in a cost-effective way.
Field Position:(80% Travel and 20% Office/Hub) located in Florida to travel for community outreach
Education: High School diploma or GED. Requires 1 – 2 years of related experience
License/Certification: Not Applicable

Care Coordinator (Level II-IV)
Position Purpose: Supports the care team by coordinating services, ensuring continuity of care, and conducting member outreach via phone or home visits.
Telephonic, Hourly Position:(25% Travel for meetings in Office/Hub) located in Florida. Receives inbound calls from members/providers seeking Care Coordination for Medicaid members.
Education: Varies based on level; Minimum Bachelor’s degree in nursing, social sciences, social work, or related field; One (1) year of supervised clinical experience in a human-services field.
License/Certification: Varies based on level

Care Coordinator II - First Year of Life (FYOL)
Position Purpose: Supports mothers and infants during the first 24 months by facilitating hospital-to-home transitions, providing family support, and ensuring access to medical and social services.
Telephonic Remote Role: Hourly position located in Florida. Answer incoming calls and conduct proactive outreach to mothers and caregivers, providing timely assistance, education, and care coordination.
Education: Bachelor's degree in nursing (BSN), social work, or a related healthcare field
License/Certification: For RN Care Coordinators, an active and unrestricted registered nurse (RN) license is required.

Birth Event Care Coordinator (Level II)
Position Purpose: Assist the Health Plan and Care Managers to obtain members missing birth event, verify birth details, and ensure timely updates to the system to ensure members have follow-up care.
Telephonic Remote Role: Hourly Position in Florida that ensures birth events for members are recorded
Education: Experience or training as a doula, lactation educator, or community health worker may be preferred by some employers.
License/Certification: Not Applicable
