Care Navigator
Position Purpose: Help members access quality care by creating personalized care plans, connecting them to resources, and improving health outcomes.
Hybrid Role: Collaborate with care managers, social workers, providers, and benefit specialists to ensure services are aligned and barriers to care are removed.
Education: Bachelor's Degree and 2-4 years of related experience
Care Navigator, Obstetrics (OB/GYN)
Position Purpose: Support pregnant and postpartum members by navigating the healthcare system, ensuring timely care, and addressing barriers like transportation, access, and social needs.
Hybrid Role: Partner with OB Care Managers, providers, hospital discharge planners, behavioral health teams, and community partners/agencies to coordinate seamless care and connect members to services like WIC, transportation, housing, food, and childcare.
Education: Master's in Behavioral Health or Social Work, or a Nursing Degree and 2-4 years of related experience
License/Certification: Licensed Master’s-level BH professional (e.g., LCSW, LMSW, LMFT) or RN with BH experience.
