Provider Engagement Account Manager
Position Purpose: Maintain partnerships between the health plan and the contracted provider networks serving our communities. Build client relations to ensure delivery of the highest level of care. Engage with providers to align on performance and consultative account management and accountability for issue resolution.
Education/Experience: Bachelor’s degree in related field or equivalent experience. Two years of managed care or medical group experience, provider relations, quality improvement, claims, contracting utilization, or clinical operations. Project management experience at a medical group, IPA, or health plan. HEDIS/Quality measures, cost and utilization.
Additional Requirements: This is a remote role with potential for local travel. Candidate must reside in Northern Nevada. Candidate with strong claims background highly preferred.
Housing Management Specialist
Position Purpose: Ensure housing compliance and help members secure permanent housing by coordinating services with providers and community partners.
Education/Experience: Bachelor’s degree in health care related field. 3+ years experience in servicing adults with SMI or in a health care related field.
Licenses/Certifications: Valid state driver’s license required.
Additional Requirements: Experience working with homelessness, and substance abuse. Willing to travel up to 75% in: Elko, Carson City, Douglas, Lyon, Storey, Churchill, and Nye County- Pahrump.
Care Navigator
Position Purpose: Coordinate and support care planning to improve health outcomes through education, and collaboration with members and providers.
Education/Experience: Requires a Bachelor’s degree and 2 – 4 years of related experience.
License/Certification: For Nevada SilverSummit (Medicaid) Healthplan: Licensed Bachelor's or Master's Prepared Social Worker: LSW or LMSW required.
Additional Requirements: This is a remote role with potential for local travel. Candidate must reside in the state of Nevada, Clark County or Washoe County residency.
Community Resource Coordinator
Position Purpose: Connect members to community resources and support care teams by providing outreach, education, and non-clinical assistance to improve access to care.
Education/Experience: High school diploma or GED required. 1–2 years of related experience preferred.
License/Certification: Valid driver’s license required. Community Health Worker certification preferred.
Location Requirements: Must reside in Elko County, Nevada or Fernley, Lyon County, Nevada. Travel up to 75% required for organizing community events, visiting homeless centers, and conducting home visits as necessary.
Complex Discharge Planner (Nurse or Social Work)
Position Purpose: Coordinate safe, effective transitions for high-risk members from acute care to next-level services by assessing needs and collaborating with providers, hospitals, and community partners.
Education/Experience: Bachelor’s or graduate degree in nursing or social work. 4+ years nursing/social work experience, including 1+ years in care coordination or discharge planning.
License/Certification: Nevada RN license or social work license (LMSW, LISW, LCSW, or LCW) required.
