Onsite Behavioral Health Care Manager
Position Purpose: Support patients as they transition from hospital to home, ensuring safe, coordinated, and cost-effective care.
On-Site Opportunity: Collaborate with providers, discharge planners, and care teams to improve outcomes and connect members to the resources they need.
Education: Master’s degree in Behavioral Health, Social Work, or Nursing, plus 4–6 years of related experience
License/Certification: Licensed Clinician (LISW, LCSW, LMSW, LMFT, LMHC, LPC, or RN required)
Pay Range: $73,800.00 - $132,700.00 per year
