LTSS Service Care Manager
Position Purpose: Coordinate holistic, long-term care management services to support quality and cost-effective healthcare outcomes; includes up to 90% travel.
Education: Bachelor’s degree and 2–4 years of related experience (or equivalent experience)
License/Certification: Valid driver's license required
Key Duties: Assess member needs and develop personalized care/service plans. Coordinate with providers, specialists, and community resources. Educate members and caregivers on services, benefits, and procedures. Monitor care plans and recommend updates as needed.

LTSS Service Care Coordinator
Position Purpose: Support the coordination of holistic, long-term care management activities to promote quality, cost-effective healthcare outcomes.
Education: Bachelor’s degree and 1 year of related experience (or equivalent experience)
Key Duties: Assess member needs and help develop personalized care/service plans. Coordinate services with families, caregivers, providers, and community resources. Monitor and update care plans; ensure accessibility and compliance. Provide resource support and education to members and their families/caregivers. Document care activities and recommend improvements to enhance service delivery

LTSS Service Care Manager (RN)
Position Purpose: Provide care management for high-risk, high-acuity members with complex medical/physical health needs to support quality, cost-effective outcomes.
Education: Graduate of an accredited School of Nursing or Bachelor’s degree with 2–4 years of relevant experience
License/Certification: RN or NP with current state licensure (or compact state licensure)
Key Duties: Conduct medical, behavioral, and functional assessments; develop and monitor individualized care plans. Coordinate with families, providers, and community resources; act as a liaison and advocate. Perform frequent in-home or on-site visits; manage skilled care and long-term nursing service needs. Educate members and caregivers on treatment, preventive care, medications, and available services. Support authorization and coordination of services (e.g., housing, transportation, ADLs)

Supervisor, Care Management LTSS
Position Purpose: Supervise a care coordination team serving long-term care members, ensuring quality, compliance, and effective care delivery.
Education: Bachelor’s degree and 4+ years of related experience (or equivalent experience)
Key Duties: Oversee team workflows, assign caseloads, and manage escalated or complex care issues. Ensure compliance with federal/state regulations and documentation standards. Provide coaching, performance evaluations, and support for staff development. Support hiring, onboarding, training, and communication between care teams, members, and providers

LTSS Service Care Manager, Behavorial Health
Position Purpose: Coordinate and deliver holistic care management for members with significant mental and behavioral health needs to support quality, cost-effective healthcare outcomes.
Education: Master’s degree in Mental Health or Social Work, or graduation from an accredited School of Nursing with 2–4 years of related experience
License/Certification: Licensed Behavioral Health Professional (e.g., LCSW, LMFT, LMHC, LPC) or RN with behavioral health experience, based on state requirements
Key Duties: Assess complex mental and behavioral health needs to develop individualized care plans. Act as liaison among members, families, providers, and community resources. Conduct home/site visits and provide crisis intervention as needed. Coordinate service authorizations and access to medical, behavioral, and social services. Educate on treatment options, medications, and preventive care. Ensure accurate documentation and compliance with clinical and regulatory standards.
