Manager, Long Term Care Program

Position Purpose: The Manager Long Term Care Program leads the design, development, implementation and oversight of a nursing facility oversight program in collaboration with internal and external stakeholders to assure that members receive safe, high quality and cost effective services.

  • Monitors and actively manages program quality, service and financial performance, including leading performance improvement activities in partnership with both internal and external stakeholders.
  • Hires, supervises, trains and manages staff interface with members, nursing facilities and interacts with internal departments and other external parties.
  • Develops the processes and curriculum necessary to train and implement models of care and program management.
  • Develops and maintains relationships with nursing facilities, advocacy organizations, support services providers and other internal and external stakeholders to effectively manage the program.
  • Researches and designs models and oversight programs to effectively manage long term care services.
  • Trains both internal and external stakeholders on programs and underlying processes.
  • Supports the facilitation and resolution of complex issues to address the needs of members.
  • Other duties as assigned.

  • Qualifications:

    The Manager acts as the primary liaison with the Long Term Care Ombudsman, Community Care Transition providers and other advocacy and support services organizations to partner in delivering care in the community in a safe and member-centered manner.

    The Manager also supports the organization’s efforts in testing new models of care and other innovations by collaborating with internal leaders and external stakeholders in establishing and monitoring the processes and communications necessary to effectively facilitate the delivery of health care services in nursing facilities or in the community, as appropriate.

    Education/Experience: Bachelor’s degree in Healthcare Management, Business Administration Public Policy or other related fields; Master’s Degree preferred. At least seven years of healthcare experience, preferably in the long term care industry with a minimum of four years of supervisory and progressively expanding roles in management
