Description:

Position Purpose: Assist the health plan in establishing a presence in the community through home visits and community outreach.

  • Serve as the lead for new Member Connections Representatives, including training and attending home visits with the Representatives
  • Assist with developing and communicating the daily schedules and routes of assigned home visits with MapPoint
  • Identify potential high risk members and reach out to case managers for possible outreach attempts
  • Enter and maintain team productivity log on intranet site
  • Pull various data, including member demographics, from various systems to assist with organizing community events
  • Educate members on health care benefits through home visits and outreach
  • Assist members with activities related to accessing care
  • Document information and activities in applicable systems
  • Deliver Connections Plus(r) cell phones, teach and answer questions related to cell phone use and navigation. Assist with auditing of active devices
  • Coordinate and implement community events, including Start Smart Baby showers and Birthday Parties, school events, healthy life style events and various community initiatives
  • Schedule and complete home visits for noncompliance, benefits informational, ED and readmission diversion and other high risk issues
  • Follow up with appropriate parties on status of high risk member referrals
  • Ability to travel throughout designated region

  • Qualifications:


    Education/Experience: High school diploma or equivalent. Bilingual ability preferred. 4+ years of social services, community outreach, or education experience.

    License/Certification: Valid driver's license and automobile insurance. Some locations may require Community Health Worker certification, CPR certification, and/or bi-lingual.
